Clinical Services

The Day Hospital strives to provide holistic care to persons who are 60 years and over on an outpatient basis. Patients are referred to the Day Hospital by their GP (Family Doctor) or by a Health Centre Doctor. They may also require further rehabilitation following a period of inpatient stay at Karin Grech Hospital. Each patient’s medical condition, rehabilitation requirements and treatment are dealt with in an interdisciplinary team approach.

 
The Nurses at the Day Hospital have successfully adopted the “Primary Nurse” system of care delivery, which is unique in the country. From their first visit, Day Hospital patients are designated to a particular nurse who will be responsible for the patient’s nursing care programme until discharge.
 
Nurses work with the Day Hospital Doctors, Consultant Geriatricians and the Psychiatrist to monitor patients’ medical and Psychiatric conditions, as well as with Physiotherapists, and Occupational Therapists. They also liaise with the Social Worker, the Speech Language Pathologist, the Psychologist, the Podiatrist and Pharmacist as necessary. Nurses also work with the Community Liaison Nurses when home visits are required.
