Renewal of cards

Diabetes Treatment can be approved free of charge on a Schedule II Card (Pink Card). However, in order to be issued a Pink Card by the Social Services, a form must be completed by a doctor at the Diabetes Clinic. This document is only issued to those patients who attend the Diabetes Clinic. Due to the long appointments waiting list, for the Diabetes Clinic, the forms are currently being filled beforehand to first time patients. The patients are referred to the clinic by their general practitioner or by a doctor who serves at a Health Centre who should complete a referral, specifying the treatment that the patient takes.
Schedule V, Schedule II and DTC permit renewals and enquiries are managed by Ms. Rose Zarb (Senior Clerk).
