Community Service


The Domiciliary Physiotherapy Service is part of the CommCare Unit. It works within its multi-professional framework to provide care in the client’s community.
The purpose of domiciliary physiotherapy is to support adult and elderly clients to live as independently as possible within their homes, maximize their quality of live and be an active part of society. An important aspect of this service is support and advice to carers. As part of the Commcare multi-professional team we work together with the client, carers and community services to promote self-care.
Aims of the service
  • To provide a h​olistic and comprehensive assessment, identify the needs and together with client and carer draw up a plan of management.
  • To clients who are best seen in their own homes, as problems linked to the living environment can be more easily identified and managed.
  • To support the carer in their caring role by advice and education in efficient moving and handling techniques.
  • To provide an efficient and effective physiotherapy service which facilitates a return to maximum functional ability following impairment. This is achieved by assessment, treatment, advice and education.
  • To provide physiotherapy to clients who are unable to leave their own home because of severe frailty or when their current health condition makes movement from their home inappropriate or difficult to client/carer (including architectural barriers e.g. flights of stairs)
  • Domiciliary physiotherapy may help reduce carer fatigue through support and consequently prevent crisis admission to secondary care and delay admission to long term care.
Procedure to request domiciliary physiotherapy services 
Requests will be received from
  • General Practitioners
  • Other health professionals from within acute and rehabilitation hospitals
  • Outpatients clinics
  • Social workers
Individuals who think that they or their relat​ives would benefit from domiciliary physiotherapy are asked to consult their general practitioner.
