Chronic Disease Management Clinic

​Primary HealthCare launched a pilot project, entitled ‘Chronic Disease Management Clinic’ (CDMC), at Qormi Health Centre on Monday 7th July 2014. 

This new clinic caters for long-term conditions such as high blood pressure, high blood cholesterol and asthma, amongst others. Diabetes is already being managed in a long-established clinic which will continue to function independently. CDMC is run by a doctor and a nurse and also deals with risk factors which contribute to chronic disease such as unhealthy eating, obesity, insufficient physical activity, excess alcohol consumption and smoking. Service users are given information regarding main topics on health promotion with emphasis on a healthy lifestyle to prevent or control these long-term conditions. 

Concurrently, CDMC also has the function of a ‘one-stop shop’. Various fragmented services which are presently in place, such as prescriptions and blood investigation results, are being integrated and delivered in this clinic.

CDMC has been extended to the following Health Centres: B'Kara, Cospicua, Floriana and Rabat.
