​This section forms an integral part of the daily operations of this Unit and the staff continuously strives to meet and exceed clients’ expectations at all times. In order to facilitate response, this Unit has issued subject-specific colour-coded forms which must be duly filled in by the patients upon receipt and returned as required.

In general the enquiries concern the following: 

  • Change in the choice of pharmacy 
    Change in Pharmacy Consent Form

  • Queries regarding new applicants
